How to Make a Check Sheet?

→ Now we will learn about the preparation of the checksheet in easy steps.

⏩ The five steps to create a check sheet are:

  1. Establish what information is needed.
  2. Determine 5W and 2H.
  3. Construction.
  4. Testing.
  5. Take the final approval for Use.

5 Steps For Check Sheet Making

Step 1. Establish what information is needed

→ First of all we need to define what information is needed.

→ Sometimes this is known as record form, form, data sheet, etc...


Step 2. Determine 5W 2H:

→ After determining the necessary information, we will conduct the 5W2H analysis.

→ Why is the data needed?

→ What type of information collection is required?

→ From where the data will be collected? (Plant/machine/place)

→ Who will collect the data?  (Operator/ supervisor/manager)

→ When will the information be collected?  (Every hour/ at the time of receipt)

→ How should the data be measured? (Instrument/clock)

→ How much data is essential?


Step 3. Construction of the Sheet:

⏩ CheckSheet has three types of information that are:

  • Title
  • Source Information
  • Content Information

Now we will learn all 3 types of information in detail.

(1) Title:

➝ In the title, we need to give a suitable title to our checksheet.

➝ Title should be clear and concise so all relevant persons can easily understand.

(2) Source information:

➝ The source information is the most important information that we need to collect for our further analysis and record.

⏩ The source information is:

  • Name of the project/ problem theme.
  • Location of data collection.
  • Date and/or time.
  • Name of the person recording data.

(3) Content information:

➝ After title and source information the last thing is the content information that we need to add.

⏩ The content information is:

  • Column/Raw with defect/event name.
  • Then Column/Raw to record frequency.
  • And Column/Raw to record the total.


Step 4. Testing of Sheet:

➝ Now the next step is testing the sheet.

⏩ Refer to the below-mentioned key criteria of testing:

  • Test the sheet with at least three different collectors.
  • Reconfirm that the columns/ rows are relevant and required.
  • Check if the sheet is easy to record and use.
  • Make changes based on the feedback.


Step 5. Final approval for using this sheet:

➝ Now the final step is to get the final approval for use or implementation.

⏩ Key points of final approval of the checksheet are:

  • Explain the use of this sheet to the data collector.
  • Ensure that the data collector understands the reason for the data is being collected.
  • Collect data.
  • Standardize its use to assure consistency.


Example of Check Sheet:

➝ Now we will take one example for a better understanding of data collection.

➝ For that we will take an appropriately designed checksheet.

➝ This sheet includes key points such as the frequency and location of defect.

⏩ Most common points that should be included in the checksheet are:

  • Operator Name
  • Type of the data that needs to be collected
  • A location where the data is being collected
  • Time of data collection
  • The objective of data collection

➝ For a better understanding refer to the below picture.

➝ In this example, we have counted the frequency of various defects at the time of checking the product in ongoing production.

➝  And at the end of the week we will analyze the defect types and will take appropriate action to eliminate those defects.

➝ In this example, we have counted the frequency of various defects at the time of online inspection of the product during manufacturing.

➝ Now we will understand how the checksheet is easily integrated with the other 6 quality control tools.

➡️Cause and effect diagram: Data from checksheets can be used to prepare cause and effect diagrams for the identification of potential causes of defects.

➡️Pareto Chart: The categorized data should be collected and used with Pareto charts to identify the most impacting factors on the quality of a product or service.

➡️Histogram: We can easily display checksheet data into the histogram for easy visualization and identification of the patterns or trends.

➡️Scatter Diagram: Collected data can be used to create scatter diagrams to identify relationships between variables.
